What changed for 2026 - and why it's worth 10 minutes

The enhanced premium tax credits that lowered Marketplace premiums the last few years expired December 31, 2025. If your household income is above roughly 400% of the federal poverty level, the subsidy cliff is back - above that line, the premium tax credit drops to zero. Below it, most households are paying a bigger share of the bill than they did last year.

Nationally, the average premium after tax credits rose about 58% - from roughly $113 to $178 a month. Options have narrowed, too: Aetna left the Marketplace for 2026, and Cigna is exiting the individual Marketplace nationwide for 2027.

If you haven't had your coverage reviewed since last year, you could be overpaying, underinsured, or both - and you won't know until someone runs your numbers. Had a life change - a new job, a move, lost coverage, a new baby, or marriage? You may qualify for a Special Enrollment Period right now.

2027 Open Enrollment opens November 1. Here's what changed.

Your plan from last year is not your plan this year. Prices moved, carriers moved, and the extra subsidy help many families counted on ended after 2025. Don't wait for a renewal letter to tell you.

The dates that matter:

  • November 1, 2026: Open enrollment starts (HealthCare.gov, Nevada Health Link, Georgia Access).
  • December 15, 2026: Enroll by this date for coverage starting January 1, 2027.
  • January 15, 2027: Open enrollment ends. Sign-ups after December 15 start February 1.

What's different for 2027:

  • Nevada: Approved rates on Nevada Health Link are up an average of 17% before subsidies, across nine carriers.
  • Georgia: Approved rates are up an average of about 21% before subsidies. Cigna and UnitedHealthcare are leaving Georgia Access after 2026, and Antidote Health is joining. If you're on Cigna or UnitedHealthcare, you'll need to pick a new plan.
  • Subsidies: The enhanced premium tax credits ended after 2025. Help is still available, but it covers less for many households. Whether you qualify depends on your income and household.
  • Renewals: Some current enrollees may be asked to confirm their information to keep their financial help.

Here's the truth: the cheapest plan on the screen is not always the best plan for your income. A low premium with a high deductible can cost you more the first time you need care. Compare the full picture: premium, deductible, your doctors, your prescriptions.

What is individual health insurance?+
Coverage you buy for yourself or your family - either through the ACA Marketplace (where income-based subsidies may be available) or as a private off-exchange plan (no subsidies, with different networks and underwriting). It's a fit if you're self-employed, between jobs, an early retiree not yet on Medicare, or without an affordable employer plan.
ACA vs. private health insurance - what's the difference?+
Both cover medical care, but they differ in pricing and access. ACA Marketplace plans offer income-based subsidies and guaranteed issue - no medical underwriting - across standardized Bronze–Platinum tiers. Private off-exchange plans can offer different networks and year-round enrollment, but may ask health questions. If you qualify for a subsidy or have pre-existing conditions, ACA is almost always the better fit.
How do ACA subsidies work, and will I qualify?+
Your household size and estimated income set a monthly tax credit (premium tax credit) that lowers your premium - sometimes dramatically. Eligibility is determined during Open Enrollment or after a qualifying life event. If your income is roughly 100–250% of the federal poverty level, you may also qualify for cost-sharing reductions on Silver plans, which lower your deductibles and copays.
Can I keep my doctor on an ACA or private plan?+
It depends on each plan's network. Lisa checks your doctors and prescriptions first, then shortlists only the ACA and private plans that include them - so you're choosing from plans you can actually use. It's always smart to confirm directly with your provider's office before you enroll.
Deductible, coinsurance & out-of-pocket max - how do they work?+
You pay your deductible first, then share costs (coinsurance and copays) until you reach your out-of-pocket maximum - after that, your plan pays 100% for covered care. A supplemental gap plan can help offset big bills with cash benefits.
